Living Well Transitions is a transitional independent living facility designed to help young adults transition from earlier patterns of destructive behavior to a supportive, successful balanced life. Some of the services that Living Well Transitions provides are individual therapy, family counseling, crisis management, life skills training, vocational and educational counseling, social skills training and substance abuse counseling. Living Well Transitions is different than a residential therapy program because participants live in their own apartments but receive constant support from professionally trained staff.
